Two trams collided in Rive on Saturday, causing major public transport disruption.
The TPG says a combination of mistakes led to the crash.
The historic green tram was running on a nearby track – which meant the points had to be changed manually.
But a regular tram passed through before the points could be changed back, so it sideswiped another.
Only one person was slightly injured by broken glass.
